Supported Living
​HIGH QUALITY CARE
The supported living scheme helps individuals with autism, mental health, learning and physical disabilities, and associated complex needs to live independently as possible in the community. We give the service user an opportunity to choose where and how they want to be supported. We focus on individual based outcomes to allow individuals to grow and develop. Things like personal dignity, control over decision making, quality lifestyle, social and leisure, financial security, accommodation and relationships are the individual based outcomes we focus on.
We work with adults aged 18 or over, who have a range of learning and physical disabilities and complex needs.
All care and support is tailored to each person, including a structured care and support plan, which uses the Life Star model. The plan encourages each person to build on their skills and strengths so they can live as independently as possible, with as much or as little support as they require.
Our supported housing schemes are for people who, with our support, will be ready to live independently or move on to alternative supported housing within six months to two years. Support available may include:

Maintaining a tenancy and dealing with correspondence

Managing finances, including budgeting and benefits

Daily living skills, including cooking, cleaning and shopping

Socialising and community involvement

Accessing education, training and employment
People who have higher-level or complex needs, who are unable to maintain their own tenancy, can receive more intensive care and support in our CQC registered care or nursing homes. Staff are on-site 24 hours a day to provide care, support and assistance, including:

Managing personal care and medication

Managing nutrition and hydration needs

Maintaining appropriate behaviour and good communication

Building positive relationships

Developing hobbies and interests
